Vibrio parahemolyticus bacteremia is rare, but can cause severe infections in immunocompromised individuals, including those with adrenal insufficiency. Prompt treatment with antibiotics and surgical debridement is crucial for survival.

Background:

  • Vibrio parahemolyticus (V. parahemolyticus) is a marine bacterium causing diarrhea and soft tissue infections.
  • Bacteremia due to V. parahemolyticus is rare, with only 11 prior reported cases.
  • Most cases involve seafood ingestion and occur in immunocompromised patients (e.g., leukemia, cirrhosis).

Observation:

  • A 60-year-old male with chronic hepatitis C and adrenal insufficiency developed V. parahemolyticus bacteremia after consuming seafood.
  • The patient presented with bacteremia, soft tissue infections (neck, leg), and multiple organ failure.
  • This represents the 12th reported case globally and the second in Taiwan.

Findings:

  • Successful treatment involved intravenous ceftazidime, oral doxycycline, and surgical debridement.
  • The patient survived despite severe complications.
  • This case highlights V. parahemolyticus as a potential pathogen in patients with adrenal insufficiency.

Implications:

  • Individuals with immunosuppression or adrenal insufficiency should avoid raw seafood and ensure thorough cooking of all seafood.
  • Clinicians should suspect V. parahemolyticus in at-risk patients presenting with fever, diarrhea, or soft tissue infections post-seafood consumption.
  • Utilizing Vibrios selective media for cultures is recommended for accurate diagnosis.
