Area of Science:

  • Epidemiology
  • Virology
  • Public Health

Background:

  • Hepatitis A virus (HAV) outbreaks pose significant public health challenges, particularly in institutional settings.
  • Early detection and control are crucial to prevent widespread transmission.

Observation:

  • An outbreak of hepatitis A occurred in a Unit for Learning Disabilities, originating from a sub-clinical case.
  • Initial control measures, including immunoglobulin, were met with resistance.

Findings:

  • Sub-clinical HAV infection was significantly associated with children under 5 years old (RR = 6.07) and a specific classroom (RR = 6.21).
  • Hepatitis A vaccination was administered to non-immune individuals, and its effectiveness in outbreak containment was assessed.
  • A total of 31 cases (18 clinical, 13 sub-clinical) were identified; no staff infections occurred.

Implications:

  • This study highlights the importance of identifying risk factors for sub-clinical HAV infection in institutional outbreaks.
  • The use of salivary antibody testing and vaccination strategies are discussed as control measures.
  • Findings underscore the need for targeted interventions in vulnerable populations during infectious disease outbreaks.
