Summary

Pyramidal anterior polar cataracts in children are congenital and may lead to amblyopia. Early monitoring for progression and prompt cataract surgery are crucial for visual outcomes.

Area of Science:

  • Ophthalmology
  • Pediatric Ophthalmology
  • Genetics

Background:

  • Pyramidal anterior polar cataracts are congenital lens opacities.
  • They may present unilaterally or bilaterally, symmetrically or asymmetrically.
  • Histologically, they comprise hyperplastic lens epithelium within a collagenous matrix.

Purpose of the Study:

  • To document the clinical characteristics of pyramidal anterior polar cataracts in children.
  • To outline the management strategies employed for these cataracts.
  • To correlate clinical findings with histopathological examination.

Main Methods:

  • Retrospective case series of 15 pediatric patients.
  • Ophthalmic examinations included visual acuity, motility, refraction, and anterior/posterior segment assessment.
  • Clinicopathologic correlation involved light and electron microscopy of retrieved opacities.

Main Results:

  • 24 eyes of 15 patients were analyzed; 9 bilateral, 6 unilateral.
  • 20 eyes developed significant cortical opacification beyond the pyramidal lesion.
  • 19 eyes required cataract surgery; amblyopia developed in most patients, often persisting despite treatment.

Conclusions:

  • Pyramidal anterior polar cataracts are congenital and may be a variant of anterior polar cataracts.
  • High likelihood of amblyopia development due to unilateral or asymmetric bilateral presentation and surrounding opacification.
  • Close follow-up for cataract progression and amblyopia is essential in infants and young children with this condition.
