A rare case of primary abdominal pregnancy occurred in a woman using an intrauterine contraceptive device (IUCD). This life-threatening condition presented as hemorrhagic shock from a ruptured ectopic pregnancy.

Area of Science:

  • Reproductive Medicine
  • Gynecology
  • Obstetrics

Background:

  • Ectopic pregnancy is a significant cause of maternal morbidity and mortality.
  • Intrauterine contraceptive devices (IUCDs) are highly effective but associated with a slightly increased risk of ectopic pregnancy if pregnancy occurs.
  • Primary abdominal pregnancy, where the gestational sac implants within the peritoneal cavity, is exceedingly rare.

Observation:

  • A 22-year-old woman, a primipara using an IUCD, presented with hemorrhagic shock.
  • Laparotomy revealed a ruptured ectopic pregnancy with over 2 liters of hemoperitoneum.
  • The products of conception were implanted on the posterior uterine surface near the right uterosacral ligament, with normal tubes and ovaries.

Findings:

  • This case represents a primary abdominal pregnancy.

Related Experiment Videos

  • The pregnancy was associated with the use of an intrauterine contraceptive device.
  • The implantation site was unusual, located on the posterior uterus rather than within the fallopian tubes.
  • Implications:

    • This case highlights the importance of considering ectopic pregnancy, even rare forms like abdominal pregnancy, in women using IUCDs presenting with shock.
    • Early diagnosis and prompt surgical intervention are critical for managing life-threatening hemorrhage in abdominal pregnancy.
    • Further investigation into the mechanisms of IUCD-associated abdominal pregnancy may be warranted.
