Summary

Depot fluphenazine and oral fluphenazine showed no significant differences in relapse rates or side effects in schizophrenia treatment. More research is needed to guide clinical practice for medication compliance.

Background:

Long-acting injectable antipsychotics (depot preparations) were developed to improve medication adherence in schizophrenia.
Concerns exist that depot antipsychotics may increase side effects compared to oral forms.

Purpose of the Study:

To compare the efficacy and safety of depot fluphenazine (enanthate and decanoate) versus oral fluphenazine for schizophrenia treatment.

Main Methods:

Systematic review of randomized clinical trials comparing depot fluphenazine to oral fluphenazine.
Searches included multiple electronic databases and reference lists.
Data extraction and quality assessment followed Cochrane Collaboration Handbook guidelines.

Main Results:

Limited data available; no significant differences found between oral and depot fluphenazine for functioning, relapse, or side effects.
Side effects like movement disorders, dry mouth, sleep issues, and weight gain were equally common.
Key outcomes such as mental state and social functioning were not adequately reported.

Conclusions:

Current evidence is insufficient to definitively guide the use of depot fluphenazine over oral forms.
The review's findings may not fully apply to real-world clinical practice where compliance is a major issue.
A large pragmatic randomized controlled trial is necessary to establish evidence-based guidelines.
