Evidence for manual therapies in asthma treatment is insufficient. More high-quality randomized controlled trials are needed to determine the effectiveness of these interventions for bronchial asthma management.

Area of Science:

  • Respiratory Medicine
  • Complementary and Alternative Medicine

Background:

  • Manual therapies are frequently used for asthma treatment, despite varied practitioners and similar proposed mechanisms.
  • A comprehensive systematic review across different manual therapy disciplines is necessary.

Purpose of the Study:

  • To evaluate the existing evidence on the efficacy of manual therapies for the treatment of bronchial asthma.

Main Methods:

  • A systematic search of multiple databases (EMBASE, CINAHL, MEDLINE, Cochrane, etc.) was conducted up to December 1998.
  • Included trials were randomized, involved asthma patients (children or adults), examined manual therapy, and reported clinical outcomes.
  • Data extraction and quality assessment were performed independently by three reviewers.

Related Experiment Videos

Main Results:

  • Five randomized controlled trials (RCTs) with 290 patients were included. Statistical pooling was not possible due to heterogeneity in patient groups and outcomes.
  • One of two chiropractic manipulation trials had good methodological quality but found no significant differences compared to sham manipulation.
  • The remaining three trials had poor quality; one suggested potential benefits of massage therapy for lung function, but results require confirmation. Chest physiotherapy and footzone therapy showed no significant differences.

Conclusions:

  • There is currently insufficient evidence to support or refute the use of manual therapies for asthma.
  • Adequately-sized RCTs with clinically relevant outcomes, observer blinding, and reporting of costs and adverse events are needed.
  • Further research is required to establish the role of manual therapies in asthma management.
