Summary

Hysterectomy generally improves quality of life and does not negatively impact sexuality for most women. Future research should consider confounding factors affecting sexual outcomes after hysterectomy.

Area of Science:

  • Gynecology
  • Women's Health
  • Quality of Life Research

Background:

  • Growing availability of alternative therapies for abnormal gynecologic bleeding necessitates re-evaluation of hysterectomy's impact.
  • Focus on hysterectomy's effect on female sexuality, a key component of overall quality of life.

Purpose of the Study:

  • To systematically review existing literature on the impact of hysterectomy on female sexuality.
  • To assess changes in libido, orgasm, and overall sexual function following hysterectomy.

Main Methods:

  • Systematic review of English and German language studies using keywords: sexuality, hysterectomy, libido, orgasm, outcomes.
  • Included 18 studies (8 prospective, 10 retrospective); assessed methodological quality despite data limitations.
  • Analyzed data using chi-squared and Fischer exact tests; noted limitations in outcome validation and confounding factors.

Main Results:

  • Most studies indicated either no change or an improvement in female sexuality post-hysterectomy.
  • Methodological quality of reviewed studies was generally poor.
  • Commonly used outcome measures were not validated, and confounding factors were often overlooked.

Conclusions:

  • The majority of research on hysterectomy and sexuality suffers from poor study design.
  • Evidence suggests hysterectomy improves quality of life and does not adversely affect sexuality for most women.
  • Future studies must account for independent confounding factors influencing sexual outcomes.
