Background:

  • Primary brainstem tumors can be challenging to resect surgically, especially when extensive.
  • Many patients with advanced brainstem tumors have undergone multiple treatments without significant improvement.

Observation:

  • Two patients with recurrent, extensive brainstem tumors presented with severe neurological deficits and required life support.
  • These patients had previously undergone biopsies, partial resections, and radiotherapy.

Findings:

  • Robotic image-guided surgery with an MKM microscope facilitated extensive tumor resection in both patients.
  • Post-surgery, both patients showed immediate improvement in motor and cranial nerve deficits.
  • One patient survived for 2 years with no tumor recurrence, while the other succumbed to pneumonia.

Implications:

  • Robotic image-guided surgery offers a viable option for previously inoperable extensive brainstem tumors.
  • This technique may improve survival and quality of life for patients with challenging brainstem pathologies.
  • Integration of surgical outlines within the microscope viewer enhances precision in complex resections.
