Summary

Biclonal chronic lymphocytic leukemia (CLL) can present without clear immunoglobulin light chain restriction. Molecular analysis revealed two distinct monoclonal B-cell populations in these challenging CLL cases.

Area of Science:

  • Hematology
  • Immunophenotyping
  • Molecular Genetics

Background:

  • Chronic lymphocytic leukemia (CLL) diagnosis relies on identifying a monotypic B-cell population (CD19+, CD5+).
  • Standard flow cytometry may not detect biclonality due to complex light chain expression.

Observation:

  • Two elderly male patients presented with absolute lymphocytosis and typical CLL immunophenotype (CD5+, CD19+, CD23+).
  • Immunoglobulin light chain restriction was not apparent by flow cytometry, with B cells expressing both kappa and lambda.

Findings:

  • Molecular genetic analysis of sorted B cells identified two distinct monoclonal populations in both patients.
  • This confirmed biclonal chronic lymphocytic leukemia (CLL) in cases initially appearing ambiguous.

Implications:

  • Biclonal CLL may require advanced molecular techniques for accurate diagnosis.
  • Understanding biclonality is crucial for precise characterization and management of certain CLL cases.
