Relapsing polychondritis (RPC) is a rare inflammatory disease that can affect the eyes. Early diagnosis of ocular manifestations is crucial for better patient outcomes.

Area of Science:

  • Ophthalmology
  • Rheumatology
  • Systemic Inflammatory Diseases

Background:

  • Relapsing polychondritis (RPC) is a rare, chronic, multisystemic inflammatory disorder targeting cartilaginous structures.
  • RPC is often associated with other autoimmune and hematologic conditions.
  • Ocular manifestations are common in RPC, including episcleritis, scleritis, and proptosis.

Observation:

  • An 83-year-old male with known RPC presented with acute unilateral chemosis, conjunctivitis, lid edema, proptosis, and extraocular muscle restriction.
  • Orbital cellulitis was excluded, and further evaluation revealed posterior scleritis with choroidal detachment.
  • The patient responded well to oral indomethacin and topical antibiotic-steroid drops.

Findings:

  • The patient's presentation and rapid response to anti-inflammatory treatment confirmed ocular complications secondary to RPC.

Related Experiment Videos

  • Ocular signs of RPC can mimic more common eye conditions.
  • The eyes can be the initial site of RPC involvement and indicate disease severity.
  • Implications:

    • Eye care practitioners must be aware of the ocular manifestations of RPC.
    • Early diagnosis and intervention in RPC ocular complications can significantly improve patient outcomes.
    • This case report and literature review aim to aid in the diagnosis and management of RPC-related eye conditions.
