Summary

Technetium-99m hexamethylpropylene amine oxime (99mTc-HMPAO) labelled leucocyte scan accurately diagnoses deep sternal wound infections (SWIs). This imaging study helps differentiate deep SWIs from superficial ones, preventing unnecessary treatments and reducing patient risks.

Area of Science:

  • Nuclear Medicine
  • Infectious Diseases
  • Surgical Infections

Background:

  • Sternal wound infections (SWIs) present diagnostic challenges, particularly distinguishing superficial from deep infections.
  • Accurate differentiation is crucial for appropriate treatment and patient outcomes.

Purpose of the Study:

  • To evaluate the diagnostic role of technetium-99m hexamethylpropylene amine oxime (99mTc-HMPAO) labelled leucocyte scan in managing SWIs.
  • To assess the scan's ability to differentiate between superficial and deep SWIs.

Main Methods:

  • A prospective study included 28 patients with suspected SWIs and 10 controls.
  • Patients were categorized based on clinical examination and suspected infection depth.
  • All patients underwent 99mTc-HMPAO labelled leucocyte scintigraphy and bacteriological examination.

Related Experiment Videos

Main Results:

  • Clinical and microbiological examination showed 68.7% sensitivity and 100% specificity for deep SWI.
  • 99mTc-HMPAO labelled leucocyte scan demonstrated 100% sensitivity and specificity for deep SWI.
  • The scan's accuracy in diagnosing deep SWI prevented unnecessary treatments for superficial infections.

Conclusions:

  • 99mTc-HMPAO labelled leucocyte scan is a valuable tool for accurately diagnosing deep SWIs.
  • The imaging modality helps avoid overestimation of infection depth, thus preventing unnecessary surgeries and antibiotic use.
