Summary
Vascular permeability/vascular endothelial growth factor (VPF/VEGF) secreted by mast cells, not epithelium, may contribute to nasal polyp formation. This finding offers new insights into the pathogenesis of nasal polyps.

Background:
- Nasal polyps are characterized by tissue edema, but their exact cause remains largely unknown.
- Vascular permeability/vascular endothelial growth factor (VPF/VEGF) is a key mediator of angiogenesis and capillary permeability.
Purpose of the Study:
- To investigate the expression and localization of VPF/VEGF in nasal polyp tissues compared to healthy nasal mucosa.
- To determine the role of VPF/VEGF in the pathogenesis of nasal polyps.

- Immunohistochemical staining was used to analyze VPF/VEGF expression in nasal polyp biopsies (n=39) and healthy nasal mucosa (n=10).
- Expression levels were compared between epithelial and mast cell compartments in both groups.

- VPF/VEGF staining was weaker in the mucosal surface and glandular epithelium of nasal polyps than in normal controls.
- Strong VPF/VEGF staining was observed in a granular pattern within mast cells of two polyp patients, whose mast cells were otherwise degranulated.
- VPF/VEGF was absent in mast cells of control subjects.
Conclusions:
- While epithelial VPF/VEGF expression is not elevated in nasal polyps, VPF/VEGF secreted by mast cells may play a significant role in their formation.
- Mast cell degranulation and subsequent VPF/VEGF release are implicated in the development of nasal polyps.
