Summary

Immunoadsorption (IA) improved heart function in patients with severe dilated cardiomyopathy (DCM) and autoantibodies. This pilot study suggests IA is a promising therapy for hemodynamic stabilization in advanced DCM.

Background:

  • Advanced dilated cardiomyopathy (DCM) has a poor prognosis.
  • Immunoadsorption (IA) shows promise for autoimmune diseases with autoantibodies.

Purpose of the Study:

  • To evaluate the short-term hemodynamic effects of IA in idiopathic DCM patients with autoantibodies.

Main Methods:

  • Pilot study included 9 patients with idiopathic DCM, severe heart failure (LVEF <30%), and beta1-adrenoreceptor antibodies.
  • Immunoadsorption performed over 5 consecutive days using an immunoglobulin adsorber.
  • Polyclonal immunoglobulin substitution after the final IA session.

Main Results:

  • Cardiac index increased from 2.0 to 2.9 L/min(-1)/m(-2) (p < 0.01).
  • Stroke volume index increased from 24.0 to 35.9 ml/m2 (p < 0.05).
