Purpose of the Study:

  • To determine the prevalence of clinically silent Morton's neuroma.
  • To identify distinguishing MR imaging features of Morton's neuroma in symptomatic versus asymptomatic patients.

Main Methods:

  • Retrospective review of 85 consecutive foot MR examinations by a single radiologist.
  • MR imaging criteria for Morton's neuroma: low- to intermediate-signal-intensity soft-tissue mass in the intermetatarsal space.
  • Patients categorized as symptomatic or asymptomatic based on questionnaires and clinical discussions; surgical confirmation in 8/25 symptomatic cases.

Main Results:

  • Prevalence of asymptomatic Morton's neuroma was 33% (19/57).

  • Symptomatic neuromas were slightly larger (mean transverse diameter 5.3 mm vs. 4.1 mm for asymptomatic), with significant overlap.

Conclusions:

  • MR imaging diagnosis of Morton's neuroma does not automatically imply clinical relevance.
  • Close correlation between clinical findings and MR imaging is essential for accurate diagnosis and treatment planning.
