Depression can precede neurological symptoms in multiple system atrophy (MSA). This case study shows successful management of depression and neurological dysfunction with antiparkinsonian medications, suggesting a common underlying cause.

Area of Science:

  • Neurology
  • Psychiatry

Background:

  • Multiple system atrophy (MSA) is a progressive neurodegenerative disorder.
  • Depression is a recognized but often overlooked early symptom of MSA.

Observation:

  • A 53-year-old woman presented with depression as the initial symptom of MSA.
  • Her condition progressed to include autonomic failure, parkinsonism, and cerebellar ataxia.

Findings:

  • Treatment with L-DOPA, L-threo-DOPS, and thyroid-releasing hormone improved autonomic and parkinsonian symptoms.
  • Depression scores significantly improved on the Zung and Hamilton scales (58 to 49 and 30 to 22, respectively).

Implications:

  • This case highlights the potential for depression to be the first manifestation of MSA.

Related Experiment Videos

  • Successful management of both depression and neurological symptoms with antiparkinsonian agents suggests a shared pathophysiology.
  • Further research into the link between depression and MSA is warranted.
