Low-dose heparin in postoperative patients: a prospective, coded study.

T H Covey, L Sherman, A E Baue

Archives of Surgery (Chicago, Ill. : 1960)
    |August 1, 1975
    PubMed
    Summary

Prophylactic subcutaneous heparin injections did not significantly reduce deep vein thrombosis (DVT) rates in patients over 40 undergoing major surgery. The study found similar DVT incidence in both heparin-treated and placebo groups.

    Area of Science:

    • Surgery
    • Vascular Medicine
    • Pharmacology

    Background:

    • Deep vein thrombosis (DVT) is a significant risk following major surgery.
    • Effective prophylactic strategies are crucial to reduce postoperative complications.

    Purpose of the Study:

    • To evaluate the efficacy of prophylactic subcutaneous heparin sodium in preventing DVT in patients over 40 undergoing major operations.

    Main Methods:

    • A randomized controlled trial involving 105 patients over 40 undergoing major surgery.
    • Patients received either 5,000 IU of heparin or a placebo subcutaneously before and after surgery for eight days.
    • Deep vein thrombosis (DVT) was monitored using 125I-fibrinogen leg scanning and confirmed with phlebography.

    Main Results:

    Related Experiment Videos

    • The overall incidence of DVT was 8.6%.
    • Heparin-treated patients had a 7.5% DVT rate (4/53), while the control group had a 9.6% DVT rate (5/52).
    • No significant increase in blood loss was observed in the heparin group.

    Conclusions:

    • Subcutaneous heparin administration showed a trend towards reduced DVT incidence but did not reach statistical significance in this study.
    • Further research may be warranted to explore optimal dosing or patient selection for heparin prophylaxis.
