Background:

Macular translocation aims to reposition the macula to a healthier retinal area.
Scleral infolding and outfolding are surgical techniques to achieve macular translocation.
Age-related macular degeneration (AMD) with subfoveal choroidal neovascularization is a leading cause of vision loss.

Observation:

Comparison of scleral infolding versus outfolding techniques in human cadaver eyes.
Radial-interrupted mattress sutures for scleral outfolding were applied in a clinical case of AMD.
Measurement of internal anteroposterior retinal contour changes after each technique.

Findings:

Scleral infolding resulted in an average retinal contour decrease of 0.36 mm.

Scleral outfolding created a significantly larger average decrease of 4.61 mm (P=.0001).

Radial-interrupted mattress sutures for outfolding demonstrated greater potential for macular translocation.

Implications:

Radial-interrupted mattress sutures may offer superior macular translocation compared to circumferential sutures.
This technique holds promise for treating subfoveal choroidal neovascularization in AMD.
Long-term efficacy and safety require further clinical investigation.
