    Background:

    • Disseminated sclerosis (DS) can lead to complex respiratory complications.
    • Understanding the neuroanatomical basis of respiratory failure (RF) in DS is critical for patient management.

    Observation:

    • Analysis of 9 patients with DS revealed distinct patterns of respiratory failure.
    • Functional pulmonary tests showed dissociation between spontaneous and controlled breathing volumes.
    • Bulbar DS localization presented severe symptoms including respiratory anarchy and impaired cough reflex.

    Findings:

    • Impaired corticospinal routes correlate with altered pulmonary function tests.
    • Bulbar DS involves glossopharyngolaryngeal paralysis, medullary respiratory center dysfunction, and reduced CO2 sensitivity.
    • Aspiration complications due to dysphagia are common in bulbar DS.
    • Extracorporeal obstruction signs predict RF development well before clinical manifestation.

    Implications:

    • Early diagnosis of impending respiratory failure in DS patients is vital.
    • Prompt respiratory resuscitation can reduce mortality and promote recovery.
    • Recognizing extracorporeal obstruction signs aids in predicting RF risk.
