Area of Science:

  • Gynecologic pathology
  • Reproductive endocrinology
  • Oncology

Background:

  • Ovarian tumors can present with diverse histological features and clinical manifestations.
  • Massive edema is an uncommon finding in ovarian neoplasms, posing diagnostic challenges.

Observation:

  • A 59-year-old woman presented with a large left ovarian mass (11 x 10 x 7 cm).
  • MRI revealed a solid, unhomogeneous mass with high T2-weighted signal intensity.
  • Microscopically, the tumor comprised a cellular area and a hypocellular, edematous area (over 75% of the tumor).

Findings:

  • The cellular areas showed spindle-shaped cells with intracytoplasmic lipid and dense collagenous stroma.
  • The edematous areas contained scattered spindle or stellate cells with minimal stromal mucin.

Related Experiment Videos

  • Histological features were consistent with ovarian fibrothecoma with massive edema.
  • Implications:

    • Distinguishing this rare entity from massive ovarian edema and sclerosing stromal tumor is critical.
    • Immunohistochemistry may not be definitive; careful histological assessment and patient age are key diagnostic factors.
    • This case highlights the importance of recognizing rare presentations of common ovarian tumors.
