Summary

A resealed anterior capsulotomy flap after cataract surgery caused a distended capsular bag filled with milky fluid. Neodymium:YAG laser treatment successfully drained the fluid and cleared opacified capsules, restoring vision.

Area of Science:

  • Ophthalmology
  • Surgical Innovation
  • Biomedical Engineering

Background:

  • A 55-year-old woman underwent conventional extracapsular cataract extraction with in-the-bag intraocular lens (IOL) implantation.
  • Three years post-surgery, a previously incised anterior capsulotomy flap spontaneously resealed.

Observation:

  • The resealed flap restored capsular bag integrity, leading to distension.
  • Milky fluid accumulated within the distended capsular bag, obscuring the IOL.
  • The intraocular lens (IOL) was observed to be centrally positioned within the distended bag.

Findings:

  • Neodymium:YAG laser was utilized to address the complication.
  • The laser effectively drained the accumulated milky fluid from the capsular bag.
  • Opacified anterior and posterior capsules were disrupted by the laser, resolving the visual obstruction.

Related Experiment Videos

Implications:

  • This case highlights a rare complication of anterior capsulotomy flap resealing post-cataract surgery.
  • Neodymium:YAG laser treatment presents a viable, minimally invasive option for managing such capsular bag distension and opacification.
  • Successful visual restoration underscores the importance of timely intervention for post-operative complications.
