Background:

  • Pleomorphic adenomas, the most common salivary gland tumors, typically occur in major salivary glands.
  • Minor salivary gland tumors are less common, and their presentation with acute airway obstruction is exceedingly rare.

Observation:

  • A case of pleomorphic adenoma originating from the soft palate presented with acute airway obstruction and respiratory failure.
  • The tumor occupied the parapharyngeal space, leading to severe respiratory compromise requiring intubation and intensive care unit admission.
  • An elective tracheostomy was performed, followed by surgical excision of the tumor from its capsule.

Findings:

  • This is the first reported case of a mixed salivary tumor of the upper respiratory tract causing acute upper airway obstruction and respiratory failure.
