A rare case reveals that lower thoracic disc herniation can cause shoulder pain. Prompt diagnosis and treatment of thoracic disc herniation are crucial for resolving shoulder symptoms and neurological deficits.

Area of Science:

  • Neurology
  • Orthopedics
  • Spine Surgery

Background:

  • Shoulder pain is commonly treated with orthopedic surgery.
  • Thoracic disc herniations are rare causes of referred pain.

Observation:

  • A patient presented with chronic shoulder pain and progressive paraplegia.
  • Initial shoulder surgery provided no relief.
  • Further investigation revealed a lower thoracic disc herniation.

Findings:

  • Surgical treatment (nucleotomy) for the thoracic disc herniation resolved both shoulder pain and paraplegia.
  • This case is the first reported instance of shoulder pain associated with lower thoracic disc herniation.

Implications:

  • Chronic shoulder pain may indicate an underlying thoracic disc herniation.

Related Experiment Videos

  • Consider central nervous system causes for peripheral symptoms before peripheral surgery.
  • Physical examination of neural tissue dynamics is vital for diagnosing referred pain patterns.
