Summary

A modified double pouch (DP) reconstruction after total gastrectomy significantly improves quality of life and nutritional recovery compared to standard single pouch (SP) techniques. The DP method offers better weight maintenance and fewer gastrointestinal issues for patients.

Area of Science:

  • Surgical oncology
  • Gastrointestinal surgery
  • Patient outcomes

Background:

  • Post-gastrectomy syndromes remain a challenge despite various reconstruction methods.
  • Current techniques like Roux-en-Y and Hunt-Lawrence single pouch reconstruction have limitations in food intake and weight management.

Purpose of the Study:

  • To compare the efficacy of a modified double pouch (DP) reconstruction versus standard single pouch (SP) reconstruction after total gastrectomy.
  • To evaluate improvements in quality of life, nutritional status, and gastrointestinal function.

Main Methods:

  • A prospective, randomized trial involving 41 patients undergoing total gastrectomy.
  • Patients were assigned to either the single pouch (SP) group (n=18) or the modified double pouch (DP) group (n=23).

Related Experiment Videos

  • Outcomes assessed at 12 months included gastrointestinal function, quality of life, body weight, nutritional parameters, serum albumin, hemoglobin, and serum protein.
  • Main Results:

    • The DP group experienced fewer symptom problems compared to the SP group.
    • Patients in the DP group showed better weight maintenance.
    • Laboratory values, including serum albumin, hemoglobin, and serum protein, were improved in the DP group.

    Conclusions:

    • Double pouch reconstruction as a gastric substitute yields superior outcomes to single pouch reconstruction.
    • The developed double pouch technique significantly enhances quality of life and nutritional recovery.
    • The DP technique leads to improved functional results and greater patient satisfaction.
