Background:
The location of parenchymal hemorrhage often indicates the site of the aneurysm in subarachnoid hemorrhage.
Case Report:
We present the first case of a thalamus hematoma associated with a small ruptured (P1) posterior cerebral artery aneurysm with rapid washout of subarachnoid blood mimicking a ganglionic hemorrhage.
Conclusion:
Thalamus hematoma with subarachnoid hemorrhage points at a ruptured proximal posterior circulation aneurysm.
