Area of Science:

  • Ophthalmology
  • Mycology

Background:

  • Cataract surgery can lead to postoperative complications, including infections.
  • Fungal endophthalmitis is a rare but serious intraocular infection.
  • Penicillium species can cause opportunistic fungal infections.

Observation:

  • A 76-year-old immunocompetent woman developed redness and pain in her right eye 5 months post-cataract surgery.
  • A soft exudative mass on the nasal lower iris was observed.
  • Aqueous culture confirmed Penicillium species as the causative agent.

Findings:

  • The patient was treated with topical natamycin and intracameral amphotericin B.
  • Successful cure of Penicillium endophthalmitis was achieved.
  • Topical corticosteroids were identified as a potential risk factor for fungal dissemination.

Implications:

  • This case highlights the successful treatment of a rare fungal endophthalmitis in an immunocompetent individual.
  • Early diagnosis and appropriate antifungal therapy are crucial for managing fungal endophthalmitis.
  • Caution is advised regarding the use of topical corticosteroids in patients with suspected or confirmed fungal eye infections.
