Summary

This case study highlights a 12-year-old boy with Takayasu's arteritis and hypertension. Findings suggest a potential link between tuberculosis and Takayasu's arteritis, indicated by positive tests and bacteria found in samples.

Area of Science:

  • Cardiovascular Medicine
  • Infectious Diseases
  • Pediatric Rheumatology

Background:

  • Takayasu's arteritis (TA) is a rare, chronic inflammatory disease affecting large arteries, primarily the aorta and its branches.
  • The etiology of Takayasu's arteritis remains largely unknown, though associations with infectious agents, particularly tuberculosis, have been suggested.

Observation:

  • A 12-year-old male presented with severe hypertension (150/90 mmHg) and Grade I hypertensive retinopathy.
  • Diagnostic workup revealed a positive tuberculin test and the presence of acid-fast bacilli (AFB) in urine.
  • Angiography demonstrated significant involvement of the aorta and occlusion of the left renal artery.

Findings:

  • Despite immunosuppressive therapy (prednisolone, cyclophosphamide) and antihypertensive management, severe hypertension persisted.

Related Experiment Videos

  • Left nephrectomy was required to control the patient's blood pressure.
  • AFB were identified in the nephrectomy specimen, further supporting a potential link to tuberculosis.
  • Implications:

    • This case suggests a possible role for Mycobacterium tuberculosis in the pathogenesis of Takayasu's arteritis, particularly in pediatric cases with severe hypertension.
    • Further research is warranted to elucidate the precise relationship between tuberculosis and Takayasu's arteritis.
    • Early identification and treatment of potential infectious triggers may be crucial in managing severe Takayasu's arteritis.
