Antibiotic treatment does not always prevent acute mastoiditis, even in communities with high antibiotic use for acute otitis media. Early myringotomy may reduce complications, and cultures are vital due to diverse pathogens.

Area of Science:

  • Otolaryngology
  • Infectious Diseases
  • Pediatrics

Background:

  • Acute otitis media is commonly treated with antibiotics.
  • Acute mastoiditis can still develop despite antibiotic use for preceding acute otitis media.

Purpose of the Study:

  • To investigate the clinical progression of acute mastoiditis.
  • To identify the specific microorganisms responsible for acute mastoiditis in a population with prevalent antibiotic use for acute otitis media.

Main Methods:

  • A retrospective analysis of 223 cases of acute mastoiditis.
  • Data collected from nine academic and non-academic referral centers.
  • Bacterial cultures were obtained from 152 patients.

Main Results:

Related Experiment Videos

  • Over half of patients received antibiotics for acute otitis media before developing acute mastoiditis.
  • Streptococcus pneumoniae, Streptococcus pyogenes, and Staphylococcus aureus were common pathogens.
  • Sixteen patients experienced severe complications, including meningitis and abscesses.

Conclusions:

  • Antibiotic therapy is not a foolproof method to prevent acute mastoiditis.
  • Early myringotomy for acute otitis media may lower complication rates.
  • Culturing is essential for identifying diverse pathogens and guiding treatment, especially with rising antibiotic resistance.
