Summary

Moisturisers benefit psoriasis, but product classification is lacking. This study found moisturisers improve skin distensibility in psoriasis plaques and uninvolved skin similarly, indicating efficacy is not influenced by lesion differences.

Background:

  • Moisturisers show promise for psoriasis management, yet a classification system is absent.
  • Previous research proposed a classification based on mechanical changes in normal skin.

Purpose of the Study:

  • To investigate if mechanical differences between psoriatic plaques and uninvolved skin affect moisturiser response.
  • To evaluate the efficacy of commonly available moisturisers on psoriatic skin mechanics.

Main Methods:

  • Measured skin mechanics (distension, hysteresis) and capacitance on psoriatic plaques and adjacent skin in 17 patients.
  • Assessed moisturiser absorption using three products: a barrier cream and a gel.
  • Utilized Dermaflex for mechanical measurements and Corneometer 812 CM for capacitance.

  • Psoriatic plaques exhibited lower baseline distensibility and capacitance compared to uninvolved skin.
  • All tested moisturisers enhanced skin distensibility in both plaque and uninvolved areas.
  • Moisturiser effects on hysteresis and capacitance varied, with some products showing differential absorption in plaques.

Conclusions:

  • Despite objective mechanical differences between psoriatic plaques and uninvolved skin, moisturiser efficacy was comparable across both.
  • The clinical differences between lesional and paralesional skin do not appear to influence moisturiser effectiveness.
  • Further research may inform a classification system for moisturisers in psoriasis therapy.
