Craniotomy effectively treated calcified chronic subdural hematomas (CSHs) in children and elderly patients. These organized CSHs, often linked to shunts or brain atrophy, showed good outcomes post-surgery.

Background:

  • Chronic subdural hematomas (CSHs) can organize and calcify over time, posing treatment challenges.
  • Previous interventions like shunts or conditions such as brain atrophy may predispose to CSH development and persistence.

Observation:

  • Magnetic resonance imaging revealed a characteristic heterogeneous, web-like structure within the calcified CSH cavities.
  • Pediatric cases involved CSHs developing in the contralateral hemisphere after infant shunting for bilateral CSHs.
  • Elderly patients presented with senile brain atrophy and significant systemic comorbidities.

Findings:

  • Surgical craniotomy for the removal of organized and/or calcified CSHs yielded favorable results in all five patients.
  • The study identified potential contributing factors to calcified CSH formation, including subdural space persistence and various brain insults.
