    Background:

    • Norepinephrine potentiation may enhance motor function in Parkinson disease (PD).
    • Levodopa is a primary PD treatment, but motor fluctuations like "on-off" responses occur.

    Purpose of the Study:

    • To investigate if clonidine hydrochloride, a norepinephrine agonist, improves motor performance in Parkinson disease patients on levodopa.

    Main Methods:

    • Administered clonidine hydrochloride to patients with mild Parkinson disease and essential hypertension.
    • Also administered to three patients with Parkinson disease experiencing "on-off" levodopa responses.

    Main Results:

    • Clonidine hydrochloride achieved a significant antihypertensive effect.
    • No demonstrable improvement in parkinsonian disability was observed.

    Conclusions:

    • Potentiation of central norepinephrine mechanisms with clonidine hydrochloride did not improve motor function in this Parkinson disease cohort.
    • Further research into norepinephrine's role in Parkinson disease motor control is warranted.
