Background:

Rathke's cleft cysts are developmental abnormalities arising from the craniopharyngeal duct.
They are incidentally found in a significant percentage of autopsies and increasingly detected via CT and MRI scans.

Observation:

Nine cases of Rathke's cleft cysts were surgically treated between 1991 and 1999.
Two of these cases, treated via a transsphenoidal approach, presented with symptoms mimicking sphenoid sinusitis, prompting otolaryngologic referral.

Findings:

The study reviews the clinical, pathological, and radiological features of these two cases.
Rathke's cleft cysts can present atypically, mimicking sinonasal conditions.

Otolaryngologists should consider Rathke's cleft cysts in the differential diagnosis of sinonasal symptoms.

Increased awareness can lead to earlier diagnosis and appropriate management of these cysts, especially those with extracranial extensions.
