Summary

Prepubic urethrostomy (PPU) in cats is a salvage procedure for urinary obstruction, often successful but with significant complications. Its use should be reserved for cases where standard perineal urethrostomy is not feasible.

Area of Science:

  • Veterinary Surgery
  • Urology
  • Small Animal Medicine

Background:

  • Idiopathic lower urinary tract disease (ILUTD) and urethral trauma are common causes of urinary obstruction in cats.
  • Perineal urethrostomy (PU) is a standard surgical treatment, but can fail or be contraindicated in some cases.

Purpose of the Study:

  • To evaluate the indications and long-term outcomes of prepubic urethrostomy (PPU) in feline patients.
  • To assess the success rate and complication profile of PPU as a salvage procedure.

Main Methods:

  • A retrospective study was conducted on sixteen cats that underwent PPU.
  • Data included indications for surgery, postoperative complications, and long-term follow-up information obtained from owners and referring veterinarians.

Related Experiment Videos

Main Results:

  • Indications for PPU included failed PU (n=9), recurrent obstruction (n=5), and urethral stenosis (n=2).
  • All cats achieved a patent urethral stoma, with 6 cats showing improvement in lower urinary tract signs after dietary management.
  • Postoperative complications occurred in eleven cats, including peristomal irritation/necrosis (n=7) and urinary incontinence (n=6). Six cats were euthanized due to complications, with a mean survival time of 13 months.

Conclusions:

  • Prepubic urethrostomy is an effective salvage procedure for relieving urinary obstruction in cats when standard techniques fail.
  • Potential complications such as incontinence and stoma necrosis necessitate careful patient selection and highlight the importance of PPU as a last resort.
