Background:

  • Nasolacrimal duct (NLD) obstruction causes epiphora and can affect the ocular surface.
  • Tear film lipid layer plays a crucial role in tear stability and ocular surface health.

Observation:

  • Changes in tear film lipid interference patterns were assessed using specular reflection video recording.
  • Tear meniscus height (TMH) and precorneal tear lipid layer interference patterns were evaluated before and after dacryocystorhinostomy (DCR).

Findings:

  • Post-DCR, tear film lipid interference patterns shifted towards higher grades (indicating a thicker lipid layer) in most patients.
  • Epiphora was resolved in all patients following DCR surgery.
  • TMH decreased in most eyes after DCR, except for one case.

Implications:

  • DCR surgery appears to positively influence the tear film lipid layer composition.
  • A thicker lipid layer post-DCR may contribute to improved ocular surface homeostasis.
  • Further research is warranted to understand the long-term effects of DCR on tear film dynamics.
