Complex duodenopancreatic traumas are rare but life-threatening. Surgical interventions for severe injuries (AAST grades IV and V) show high mortality, but case studies suggest improved outcomes with tailored treatments.

Area of Science:

  • Trauma Surgery
  • Surgical Gastroenterology

Background:

  • Duodenopancreatic injuries are uncommon and challenging to diagnose and manage.
  • High mortality in these cases is often linked to associated injuries and severe trauma (AAST grade IV and V).

Observation:

  • The study reviewed 6 cases of complex duodenopancreatic trauma between 1995 and 1999.
  • Etiologies included blunt trauma (83%) and a shotgun wound (17%).
  • Treatments ranged from pancreaticoduodenectomy for pancreatic head injuries to distal splenopancreatectomy and duodenal resection.

Findings:

  • Pancreaticoduodenectomy for grade V pancreatic head lesions had a 31% postoperative mortality in a prior review.
  • In this series, overall mortality was 16%, with one death following duodenal resection.
  • A pancreatic fistula occurred in one case but resolved spontaneously.

Related Experiment Videos

Implications:

  • Complex duodenopancreatic traumas necessitate specialized surgical approaches.
  • Outcomes for severe duodenopancreatic injuries, while serious, may be improved with appropriate surgical management.
  • Further research into optimal treatment strategies for these rare injuries is warranted.
