Area of Science:

  • Cardiothoracic Surgery
  • Surgical Education
  • Patient Outcomes

Background:

  • Potential conflict between optimal patient care and surgical training.
  • Need to evaluate trainee performance in cardiac surgery.

Purpose of the Study:

  • Analyze cardiac surgery training effectiveness.
  • Assess trainee outcomes in relation to workload and risk stratification.

Main Methods:

  • Retrospective audit over six years.
  • Analysis of consultant vs. trainee procedures.
  • Utilized the Parsonnet system for risk stratification.

Main Results:

  • Trainees performed 2166 of 6037 operations, primarily CABG (88%).
  • Lower median Parsonnet score for trainee operations (4) vs. consultant (9).

Related Experiment Videos

  • Actual mortality below predicted; trainee morbidity was lowest (10%).
  • Conclusions:

    • Trainees achieve good results with appropriate case selection.
    • Supervised exposure to a wider range of procedures is crucial for training.
    • Adjustments needed in UK training to address lower surgical volumes.
