    Summary

    Phenytoin treatment significantly reduced late-onset epilepsy in severe head injury patients. Untreated patients had a 50% epilepsy incidence, while phenytoin users had only 10%.

    Background:

    • Posttraumatic epilepsy (PTE) is a significant complication following severe head injuries.
    • Identifying effective preventative treatments for PTE remains a critical clinical challenge.

    Purpose of the Study:

    • To evaluate the efficacy of phenytoin in reducing the incidence of late-onset posttraumatic epilepsy in patients with severe head injuries.

    Main Methods:

    • Retrospective study of 62 patients with severe head injuries.
    • Comparison of epilepsy incidence between patients treated with phenytoin and those not treated.

    Main Results:

    • Only 10% of 50 patients treated with phenytoin developed late-onset epilepsy.
    • Twelve untreated patients with similar head injuries experienced a 50% incidence of epilepsy.

    Conclusions:

    • Phenytoin treatment appears to decrease the incidence of posttraumatic epilepsy in a highly selected group of severe head injury patients.
    • These findings support the prophylactic use of phenytoin to prevent epilepsy after significant head trauma.
