Area of Science:

  • Reproductive Medicine
  • Pulmonology
  • Pathology

Background:

  • Endometriosis typically affects pelvic organs but can occur in rare extrapelvic locations.
  • Pulmonary endometriosis presents a diagnostic challenge, often mistaken for neoplastic lesions.

Observation:

  • A 48-year-old woman underwent surgery for a suspected pulmonary neoplasm.
  • Histological examination revealed pulmonary endometriosis with endometrial glands and stroma.
  • A second focus responded to treatment with a GnRH agonist (Zoladex).

Findings:

  • Pulmonary endometriosis can be successfully treated with gonadotropin-releasing hormone (GnRH) analogues.
  • Conservative management with GnRH agonists led to disease resolution without surgery.
  • Long-term follow-up showed no recurrence after three years.

Implications:

  • Accurate clinical and morphological diagnosis is crucial to avoid unnecessary extensive surgery for pulmonary endometriosis.
  • GnRH analogue therapy offers a viable conservative treatment option for pulmonary endometriosis.
  • Management strategies should consider both conservative therapy and surgical options, followed by diligent follow-up to prevent complications.
