Background:

  • Multiple sclerosis (MS) is a CNS autoimmune disease.
  • Rheumatic diseases like Sjögren's syndrome (SS) can mimic MS.

Observation:

  • Two patients with MS presented with xerophthalmia, xerostomia, and positive Ro antibodies.
  • Both patients exhibited neurological symptoms and brain lesions on MRI consistent with MS.

Findings:

  • Patients fulfilled diagnostic criteria for SS, despite initial MS diagnosis.
  • Elevated autoantibodies (ANA, Anti-SSA) were noted in follow-up studies.

Implications:

  • SS should be considered in the differential diagnosis of MS.
  • Neurological manifestations of SS can closely resemble MS, necessitating thorough autoimmune workup.
