Summary

Routine enemas before prostate biopsies do not improve outcomes and may increase patient discomfort. This study found no significant difference in complications between patients who received enemas and those who did not.

Area of Science:

  • Urology
  • Oncology
  • Medical Procedures

Background:

  • Transrectal ultrasound-guided prostate biopsy is standard for diagnosing prostate cancer.
  • Pre-biopsy enema protocols lack definitive assessment in existing literature.

Purpose of the Study:

  • To evaluate the clinical significance of pre-biopsy enemas in transrectal ultrasound-guided prostate biopsies.
  • To determine if enema preparation impacts complication rates.

Main Methods:

  • Retrospective review of 448 transrectal ultrasound-guided prostate biopsies.
  • Comparison of complication rates between patients who received enemas (n=225) and those who did not (n=185).
  • All patients received identical antibiotic prophylaxis (ciprofloxacin).

Related Experiment Videos

Main Results:

  • No significant difference in overall clinically significant complications (4.4% with enema vs. 3.2% without, p=0.614).
  • Similar rates of interventions (prostatectomy/resection) for hematuria/retention in both groups.
  • No infectious complications leading to hospitalization in the non-enema group.

Conclusions:

  • Transrectal ultrasound-guided prostate biopsy with quinolone prophylaxis is safe.
  • Pre-biopsy enemas offer no demonstrable clinical advantage.
  • Enema use may increase patient cost and discomfort without improving outcomes.
