Background:

  • Descemet's membrane detachment (DMD) can occur postoperatively.
  • Bilateral cases of DMD are rare and their etiology is not well understood.

Observation:

  • A case of consecutive bilateral Descemet's membrane detachments after sequential phacoemulsification is presented.
  • A review of three additional patients with bilateral DM detachments was conducted.
  • No definitive underlying etiology was identified in the reported cases.

Findings:

  • The study found no clear underlying etiology for Descemet's membrane detachment in the reviewed cases.
  • Anatomic predisposition, potentially due to abnormalities in the fibrillary stromal attachment to Descemet's membrane, is hypothesized.
  • Bilateral Descemet's membrane detachments occurred in consecutive surgical events in one patient.

Implications:

  • Patients may have an inherent anatomical susceptibility to Descemet's membrane detachment.
  • Early surgical intervention for Descemet's membrane detachment typically yields favorable visual outcomes.
  • Further research into the anatomical factors predisposing individuals to Descemet's membrane detachment is warranted.
