    Area of Science:

    • Oral oncology
    • Surgical pathology
    • Radiation oncology

    Background:

    • Pleomorphic liposarcoma is a rare soft tissue malignancy.
    • Gingival liposarcomas are exceptionally uncommon, with few documented cases.
    • Accurate histological diagnosis is crucial for appropriate management.

    Observation:

    • A 24-year-old male presented with a lesion clinically resembling a lipoma in the buccal gingival sulcus.
    • Histological examination revealed pleomorphic liposarcoma, a malignant tumor.
    • Initial treatment involved local excision and maxillectomy with orbital function preservation.

    Findings:

    • Postoperative radiation therapy (5,000 rads) was administered.
    • Recurrence of the tumor occurred four months after initial treatment.
    • The recurrent tumor was located outside the primary surgical site.

    Implications:

    • Multimodality treatment including surgery, radiation, and chemotherapy may be necessary for aggressive liposarcomas.
    • Close follow-up is essential due to the potential for recurrence.
    • This case highlights the importance of thorough histopathological evaluation for oral soft tissue tumors.
