Summary

Intravaginal prostaglandin E1 (PGE1) significantly shortens labor induction to delivery time compared to prostaglandin E2 (PGE2). PGE1 achieves this by promoting faster cervical ripening and quicker onset of active labor.

Area of Science:

  • Obstetrics and Gynecology
  • Reproductive Medicine
  • Pharmacology

Background:

  • Intravaginal misoprostol (prostaglandin E1, PGE1) shows greater efficacy in labor induction than dinoprostone (prostaglandin E2, PGE2).
  • Studies indicate PGE1 reduces the induction-to-delivery interval, but its specific effects on different phases of labor require further investigation.

Purpose of the Study:

  • To compare the time-dependent activity of intravaginal PGE1 versus PGE2 during labor induction.
  • To determine if PGE1 or PGE2 is more effective in accelerating specific stages of the labor induction process.

Main Methods:

  • A comparative study involving 155 patients receiving intravaginal PGE1 (50 micrograms) and 174 patients receiving PGE2 (3 mg).
  • Doses were repeated every 6 and 24 hours until labor onset. Key metrics included induction-to-initiation of labor, labor duration, induction-to-delivery time, and cervical ripening (Bishop score).

Related Experiment Videos

Main Results:

  • While demographic factors and maternal/neonatal outcomes were similar, PGE1 significantly reduced the induction-to-initiation of labor interval (7.37 h vs. 11.25 h, p < 0.01) and induction-to-delivery interval (11.5 h vs. 14.4 h, p < 0.05).
  • PGE1 also demonstrated a significantly greater improvement in Bishop score compared to PGE2, indicating more effective cervical ripening.

Conclusions:

  • Intravaginal PGE1 is more effective than PGE2 in reducing the overall induction-to-delivery time.
  • The enhanced efficacy of PGE1 is attributed to its superior ability to promote cervical ripening, leading to a faster achievement of active labor.
