    Summary

    This study reports on the pharmacology of a new anticancer drug, PTG. PTG shows a long terminal half-life, is extensively metabolized, and has limited blood-brain barrier penetration, with observed leukopenia in patients.

    Background:

    Purpose of the Study:

    • To investigate the pharmacology of 4'-demethylepipodophyllotoxin 9-(4,6-0-thenylidene-beta-D-glucopyranoside) (PTG) in patients with advanced cancer.
    • To determine drug recovery, metabolism, plasma decay, and blood-brain barrier penetration of PTG.

    Main Methods:

    • Six patients with advanced cancer received tritium-labeled PTG intravenously.
    • Urine and fecal excretion were measured.
    • Plasma drug concentrations were analyzed using nonlinear least-squares regression.
    • Cerebrospinal fluid (CSF) drug levels were assessed.

    Main Results:

    • Urine recovery was 44.49% within 72 hours, with 78.7% being metabolite.
    • Fecal recovery ranged from 0 to 10.05%.
    • Plasma decay followed a triphasic model with a long terminal phase half-life (11-38.5 hours).
    • CSF drug levels were <1% of plasma levels, except in one post-surgery/radiotherapy patient (27%).
    • Leukopenia (WBC <5,000/mm3) occurred in all 4 evaluable patients for toxicity.

    Conclusions:

    • PTG exhibits a prolonged terminal phase half-life and is significantly metabolized.
    • The drug demonstrates poor penetration of the normal blood-brain barrier.
    • Leukopenia is a notable toxicity, potentially related to plasma half-life.
