  • The cyst manifested as a significant space-occupying lesion.

Findings:

  • Surgical excision of the cyst wall and creation of a subdural space communication were performed.
  • Postoperatively, the cyst size was substantially reduced.
  • Marked improvement in neurological signs and symptoms, including monoparesis, was observed.

Implications:

  • Surgical intervention is an effective treatment for interhemispheric cerebral cysts causing motor disturbances.
  • Early diagnosis and surgical management can lead to significant neurological recovery.
  • This case contributes to understanding the clinical presentation and surgical outcomes of rare interhemispheric cysts.
