Summary

Gastrointestinal autonomic nerve tumors are rare and aggressive. Surgical removal is the primary treatment, as chemotherapy is ineffective for these neurogenic stromal tumors.

Background:

  • Gastrointestinal autonomic nerve tumors (GAN-tumors) are rare malignant neurogenic stromal tumors originating from the autonomic nerve plexus of the intestinal tract.
  • Interstitial cells of Cajal are suspected precursors to GAN-tumors.

Observation:

  • A 53-year-old patient presented with a 5 cm jejunal tumor, diagnosed as GAN-tumor via resection and immunohistochemistry.
  • The patient developed omental metastases and diffuse peritoneal spread within 29 months post-surgery.

Findings:

  • Adjuvant chemotherapy regimens (adriamycine/ifosamide, taxotere, gemcitabine/xyloda) were ineffective in treating the metastatic disease.
  • Tumor size appears to correlate with recurrence risk: <5 cm tumors showed no recurrence, while >10 cm tumors had a 64% recurrence rate within 2 years.
  • The patient died 15 months after the second operation due to disease progression.

Implications:

  • Surgical resection is the current treatment of choice for GAN-tumors, including cases of recurrence.
  • The lack of effective medical treatment options highlights the need for further research into novel therapeutic strategies for advanced GAN-tumors.
