Summary

The combined use of compression stockings and low-molecular weight heparin (LMWH) was most effective in preventing thromboembolic complications after major abdominal surgery. This combined prophylaxis significantly reduced deep venous thrombosis and pulmonary embolism risks.

Area of Science:

  • Vascular Surgery
  • Thromboembolic Disease Prevention
  • Postoperative Complications

Background:

  • Major abdominal surgery poses a significant risk for thromboembolic complications.
  • Effective prophylaxis strategies are crucial to improve patient outcomes.

Purpose of the Study:

  • To compare the efficacy of compression stockings, low-molecular weight heparin (LMWH), and a combination of both for prophylaxis after major abdominal surgery.

Main Methods:

  • A randomized controlled trial involving patients undergoing major abdominal surgery.
  • Three groups received: compression stockings, LMWH, or combined therapy.
  • Deep venous thrombosis (DVT) and pulmonary embolism (PE) were assessed using Doppler ultrasonography and pulmonary scintigraphy.

Related Experiment Videos

Main Results:

  • The combined therapy group showed the lowest incidence of DVT (3/92) and PE (1/92) compared to compression stockings alone (10/91 DVT, 6/91 PE) and LMWH alone (8/91 DVT, 1/91 PE).
  • Wound hematoma rates were higher in the LMWH group (8/91) compared to the combined group (3/92).
  • The combined treatment demonstrated the most favorable risk-benefit profile.

Conclusions:

  • While no single modality completely prevented thromboembolic events, combined prophylaxis with compression stockings and LMWH was the most effective strategy.
  • This approach offers a significant reduction in DVT and PE incidence post-major abdominal surgery.
