  • The lesion was characterized by a multiloculated, expansive mass with internal fluid levels on MRI.

  • MRI findings were highly suggestive (pathognomonic) of an aneurysmal bone cyst.
  • Radical surgical resection was performed for the calvarial lesion.

Implications:

  • This case highlights the possibility of rare calvarial ABCs.
  • Complete surgical excision offers a curative treatment option for these lesions.
