Valproate (VPA) add-on therapy for epilepsy can cause acute encephalopathy, even with prior safe VPA use. A high initial VPA dose may trigger this adverse reaction.

Area of Science:

  • Neurology
  • Clinical Pharmacology
  • Toxicology

Background:

  • Carbamazepine (CBZ) is a common antiepileptic drug.
  • Valproate (VPA) is frequently used as an add-on therapy for epilepsy.
  • Drug-induced encephalopathy is a potential adverse effect of antiepileptic medications.

Observation:

  • A 50-year-old woman on carbamazepine (CBZ) developed acute confusion after 10 days of valproate (VPA) add-on treatment.
  • The patient presented with elevated serum ammonia and EEG findings indicative of diffuse encephalopathy.
  • Serum levels of VPA and CBZ were within non-toxic ranges, and liver functions were normal.

Findings:

  • Discontinuation of VPA led to rapid clinical recovery.
  • Previous safe administration of VPA did not prevent the occurrence of encephalopathy in this case.

Related Experiment Videos

  • A high initial VPA dosage is suspected as the potential cause of the encephalopathy.
  • Implications:

    • Clinicians should be vigilant for signs of encephalopathy in patients receiving VPA, especially with high initial doses.
    • This case highlights that prior tolerance to VPA does not guarantee future safety.
    • Further investigation into dose-dependent VPA neurotoxicity is warranted.
