Open Angle Glaucoma: Treatment01:27

Open Angle Glaucoma: Treatment

915
In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. The treatment for open-angle glaucoma focuses on reducing the elevated intraocular pressure by either decreasing the secretion of aqueous humor or increasing its outflow.

Angle Closure Glaucoma: Treatment

1.1K
Angle-closure glaucoma, or closed-angle glaucoma, is an eye condition where the iris bulges out and blocks the iridocorneal angle, resulting in a buildup of aqueous humor and increased intraocular pressure. Immediate medical attention is necessary due to the sudden onset of symptoms. The treatment for angle-closure glaucoma includes short-term and long-term approaches. Short-term treatment involves using eye drops like pilocarpine to lower intraocular pressure by increasing aqueous humor...

Glaucoma: Overview

1.2K
Glaucoma is an eye condition characterized by increased intraocular pressure that damages the retina and optic nerve, leading to irreversible blindness if left untreated. The human eye has various components, including the cornea, iris, pupil, lens, and optic nerve. Aqueous humor is secreted by the epithelium of the ciliary body in the posterior chamber and flows through the trabecular meshwork and canal of Schlemm, maintaining normal intraocular pressure. The trabecular meshwork and the canal...

Summary

Neodymium-doped Yttrium Aluminum Garnet (Nd:YAG) laser capsulotomy effectively treats posterior capsular thickening after cataract surgery. Post-procedure intraocular pressure (IOP) elevation was observed but manageable with medication.

Area of Science:

  • Ophthalmology
  • Surgical Complications
  • Laser Therapy

Background:

  • Posterior capsular thickening affects 28-43% of patients post-extracapsular cataract surgery.
  • Neodymium-doped Yttrium Aluminum Garnet (Nd:YAG) laser capsulotomy is a primary treatment for this complication.

Purpose of the Study:

  • To prospectively evaluate the intraocular pressure (IOP) rise following Nd:YAG laser capsulotomy.
  • To assess the efficacy of medical management for post-laser IOP elevation.

Main Methods:

  • A prospective, randomized comparative clinical study was conducted.
  • 54 patients undergoing Nd:YAG laser capsulotomy were enrolled.
  • Intraocular pressure (IOP) changes were monitored post-procedure.

Main Results:

  • The mean age of patients was 63.6 years (43% male, 57% female).
  • Capsular opacification occurred at a mean of 14.6 months post-surgery.
  • All patients experienced a post-laser IOP rise, successfully managed with topical beta-blockers and steroids.
  • Significant visual improvement was noted, from 6/24 to 6/6.

Conclusions:

  • Nd:YAG laser capsulotomy is an effective treatment for posterior capsular thickening.
  • Post-laser IOP elevation is a common occurrence but effectively controlled with standard pharmacological interventions.
  • This treatment offers significant visual rehabilitation for patients.
