Background:

  • Endodermal cysts are rare intracranial tumors.
  • Typically found in the posterior fossa, they usually occur in the subdural space of the spinal cord.

Observation:

  • A 71-year-old man presented with gait disturbance.
  • Imaging revealed a cystic lesion in the quadrigeminal and right ambient cisterns.
  • The cyst was subtotally removed via a suboccipital transtentorial approach.

Findings:

  • Histopathological examination confirmed an endodermal cyst.
  • The cyst wall showed columnar epithelium and connective tissue.
  • Immunostaining supported the diagnosis of an endodermal cyst.

Implications:
